Your Workers' Compensation Declaration Page(s) includes a list of the coverage limits you currently have as well as your company's Job Codes and the assigned rates. It is often referred to as the "dec page." 

Additionally, the dec page provides vital information regarding your policy - such as your policy number, coverage period, and agent information - that we will need to add the policy to your company set-up. The dec page is typically found near the front of your workers' compensation policy and may be more than one page in length.
